Turn incoming referral documents
into clear next steps.

Move every referral forward faster: identify what arrived, flag what needs attention, and route it to the right team or system without manual sorting.

  1. 01 · Understand

    Understand what arrived

Identify referral documents and surface the patient, referring provider, service line, order, and other details needed next.

  2. 02 · Review

    Catch what needs attention

Use confidence levels, patient matching, and human review to flag information that is incomplete, uncertain, or requires judgment.

  3. 03 · Route

    Route toward care

Send the document and relevant data to the right specialty, team, work queue, or downstream process.

  4. 04 · Connect

    Connect the workflow

Deliver documents and data straight into the EHR and other systems—cutting manual data entry, saving staff hours, and moving patients to scheduling and care sooner.

Built to support the workflows and systems teams already use—not replace every process around them.

Concord Connect Ignition program

Start with one workflow. Prove the value. Scale from there.

When information stalls

Stalled referrals can create problems far beyond the intake queue.

  1. 01

    Information is missing

History, labs, notes, imaging, or prior results are not ready when the team needs to act.

  2. 02

    The referral reopens

Staff search systems, request records, re-key details, and return to the same referral again.

  3. 03

    Capacity sits unused

Appointments, beds, or equipment slots sit idle while the next step waits.

  4. 04

    The patient waits

Care slows, referral partners follow up, and the next referral may go somewhere easier.

60%

of healthcare leaders say staff spend 4+ hours on document triage and sorting.1

14%

of diagnostic tests are reordered because faxed information is missing or lost.2

Julie’s mom waited for the images her care team needed

A care transition story

What happens when information doesn’t move with the patient?

“After a fall at home, my Mom was unconscious and so was transported by ambulance to a nearby ER. There they took images and determined she had a skull fracture as well as a broken wrist. That triggered a transfer to a nearby Level I trauma center, where she could be evaluated by a neurologist.

Once there, the neurologist took new images, did not see signs of a fracture, and so requested the original images from the sending facility. It took 10 hours for those to be delivered, during which time she was kept immobilized and in a neck brace, all of which was frightening and very uncomfortable.

When the original images arrived, the neurologist determined that what had been considered a fracture was in fact a shadow, and Mom was sent home with some painkillers and a treated, broken wrist.”
